Semantic and pragmatic syndrome: dysphasia, autism or “psychotic dysharmonia”?

Abstract

`titrebSEMANTIC-PRAGMATIC DISORDER : DYSPHASIA, AUTISM OR « PSYCHOTIC DISHARMONY » ?`/titreb`!-- 447:'RESUME' --bAmong developmental disorders in children, the semantic-pragmatic disorder is a clinical condition whose nosographic definition remains ambiguous and has been debated since its introduction in 1983. The syndrome mainly concerns language pragmatics (i.e. the content and the use of language in communication). According to most current clinical views, it is included among developmental language disorders (dysphasia) although it is atypical compared to specific or structural language impairments. On the other hand, the syndrome has more similarity and co-occurrence with autism or the « autistic spectrum » but it does not meet the complete, habitual criteria for these latter categories. Three working hypotheses have been discussed concerning its nature : that it might be a subtype of dysphasia, a subtype of the autistic spectrum, or a distinct, intermediate condition between dysphasia and autism. A fourth and alternative hypothesis suggests that it could be a moderate form of another type of interaction and communication disorder like autism, currently almost unknown in English literature : « psychotic disharmony ». To clarify the existence and nature of semantic-pragmatic disorder, this paper makes an extensive review of clinical descriptions as well as experimental studies of the syndrome, and discusses the different nosographic hypotheses. On a general level, the pragmatic impairments, both verbal and nonverbal, involved in this syndrome are consistent with a communication disorder, as is the case for autism. However, a more specific analysis of the disorder reveals the prolixity and incoherence of interactions, which clearly distinguishes it from autism and suggests a hypothesis closer to that of « psychotic disharmony ».
