Support after acquired brain injury of the cranial traumatic type

Abstract

`titrebManagement following an acquired brain lesion due a head injury`/titrebTraumatic brain injury (TBI) is a major issue for Public Health, as it frequently occurs in young, healthy adults, and can result in severe permanent disability. The main causes in France are road traffic accident and falls.Most common neurological deficits for patients with TBI are cognitive and behavioural disorders, sensorimotor deficits, balance and bladder problems. There is a wide range of neurological symptoms. Nevertheless, cognitive and behavioural disorders are mainly responsible for patient loss of autonomy.Patients with TBI require specific management during acute care with multimodal monitoring in neuro-intensive care units and then within specialized departments in cases of a minimally conscious state. During post-acute rehabilitation, TBI patients should be referred to a neuro-rehabilitation unit with specific intervention programmes based on multidisciplinary rehabilitation and additional special focus on cognitive neuro-rehabilitation. Finally, patients should seek long-term follow-up with help to plan their new life including their activity limitations as well as participation restrictions. The lack of information regarding specialized departments and pathways might explain patients’ loss-to-follow-up at each stage of the therapeutic course, which might subsequently result in further devastating consequences for patients’ future life.
